Who Qualifies for Medical Cannabis Card

To legally purchase and use medical marijuana in Illinois, you must meet the following criteria:

  • Be 18 years of age or older (minors need two physician recommendations as well as consent from their legal guardian)
  • Have consent from a physician
  • Be an Illinois state resident with proof of residency
  • Be diagnosed with a qualifying medical condition

How to Get a Card

To apply for a Cannabis Card in Illinois, you must first become a medical marijuana patient. Here’s how to get started:

1. Meet with a doctor to verify your qualifying condition.

A doctor must verify that you have been diagnosed with a qualifying medical condition by signing the Physician Verification Form.

2. Complete the qualifying patient application form.

3. Mail completed forms to:

Your application will be reviewed by the Illinois Department of Public Health Division of Medical Cannabis between 30 and 90 days of receiving it. Once approved, you will receive your card within 15 days.
